Start with the scope

  1. Confirm that this is an existing slab, not a planned new pour.
  2. Name the surface and every adjoining feature.
  3. Estimate dimensions and count distinct panels.
  4. Describe visible conditions without assigning a cause.
  5. Record when the change was first noticed.
  6. State the outcome you want evaluated.

Quick slab checklist

Give the provider a clearer starting point

Write down what you can see and measure. These facts can save questions later, but they do not decide whether lifting is right for the slab.

  1. Approximate slab length, width, and thickness if known
  2. Surface material and finish
  3. Visible offsets, cracks, spalls, rocking, or pooled water
  4. When the condition was first noticed
  5. Prior work or treatments if known
  6. Overall, transition, condition, drainage, and access photos
  7. Timeline, access constraints, and property authority

Photo sequence

  • Overall surface from at least two safe directions
  • Each offset with a ruler or tape for scale
  • Cracks, joints, spalls, exposed edges, and transitions
  • Nearby downspouts, drains, slopes, and water paths
  • Access route without people, plates, or house numbers
